HealthE Dividends and the Health Risk Assessment (HRA) are two separate programs.
The HRA is an on-line questionnaire that you complete by entering your health information and biometric data. The HRA helps you identify your personal health risks. The time period to complete an HRA is September 1 - August 31. If you are married and your spouse is covered by an Eastman healthcare plan, you can lower your healthcare premium for the next year by $600 if both you and your spouse complete an HRA. If you have single coverage, you can lower your healthcare premium by $600.

If your doctor collects your biometrics, you can complete your HRA online. Just go to www.healthEconnections.net to complete the questionnaire and enter the biometric results from your doctor.

The HealthE Dividends program provides incentives for employees and covered spouses who participate in programs and services designed to help you maintain or improve your health. Knowing your health risks allows you to select the programs that will be the most beneficial to you.
