     In consideration of Eastman Chemical Company, Texas Operations permitting the Group/Class, hereafter collectively referred to as "PARTICIPANT" to enter its Nature and Wildlife Habitat Center, hereafter referred to as "ACTIVITY," I hereby release Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, and officers from all claims, demands, suits, causes of action, or judgments which I or anyone in my Group/Class ever had, now have, or may have in the future or which my or my Group/Class' heirs, executors, administrators, or assigns may have, or claim to have against Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, or officers, arising out of or in any way connected with the ACTIVITY, for all personal injuries, known or unknown, property damages, or claims for wrongful death, caused by the ACTS, OMISSIONS OR NEGLIGENCE of Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, officers, or any other party while on Eastman Chemical Company, Texas Operations' property. I, Leader of the Group/Class further agree that if any Participants are under the age of eighteen (18) years old, I assume responsibility for such Participants and have authority and do hereby execute this Agreement on their behalf.

I further agree to indemnify and hold harmless Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, and officers from all claims, demands, suits, causes of action, or judgments which PARTICIPANT ever had, now has, or may have in the future or which PARTICIPANT'S heirs, executors, administrators, or assigns may have or claim to have against Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, or officers, arising out of or in any way connected with the ACTIVITY, for all personal injuries, known or unknown, property damages, or claims for wrongful death, caused by the ACTS, OMISSIONS OR NEGLIGENCE of Eastman Chemical Company, Texas Operations, its directors, agents, employees, shareholders, officers, or any other party whether known or unknown, and further agree, on Eastman Chemical Company, Texas Operations' behalf and in Eastman Chemical Company, Texas Operations' name, to defend at my own expense any such claims, demands, suits, causes of action or judgments described above.

I also agree to be responsible for any property damage or personal injuries that PARTICIPANT may cause by his or her intentional or negligent acts while participating in the ACTIVITY. I have read and executed this document with full knowledge of its significance.
 

Visitors' Signatures (if over 18 years of age)

Visitors will be required to sign this document prior to entering the Center. If an individual(s) is responsible for a class or group of young people (under 18 years old), then this individual(s) should complete this form and sign this document on behalf of the class or group as
"Leader Responsible for Group."

Approved form to be returned to Eastman Security at Main Guard Gate.
